The Foundation makes funding decisions once a year, typically in June. Invited proposals for support must complete an online application form that includes the information below. Foundation staff will provide a link and unique login information to invited organizations. The Foundation does not accept unsolicited proposals.
- Brief description of mission, history, background and capabilities of organization
- Description of project or program, including:
- Alignment with Foundation mission and guidelines
- What will be accomplished?
- What are your strategies for ensuring project or program success?
- What are your organization’s capabilities for implementing the project or program?
- How will the program or project be sustained?
- Implementation plan with timelines
- Evaluation plan of how your organization will demonstrate progress
- Other actual and anticipated sources of support
- Detailed program or project budget
- Current fiscal year budget
- Copy of IRS determination letter
- Copy of most recent audited financial statements and/or IRS Form 990
- Listing of Board of Directors and their affiliations
- Listing of all current funders
